What is Companion Care?
Companion care is a type of in-home care that focuses on providing social interaction, emotional support, and daily assistance to individuals who are in need. What do our caregivers do?

What is care coordination?
Care coordination is a service that involves our care coordinator coordinating the various aspects of an individual’s care to ensure that they receive the most effective and efficient care possible. This is particularly important for individuals with chronic conditions, who may need to see multiple healthcare providers and specialists to manage their condition.
Our exceptional care coordinator will spend time getting to know you and be there for a family and their loved one during a time of need so that the family can focus on being a family.
What are the goals of care coordination?
We understand that it can be a trying time when a family member is declining, and our care coordinator is someone who can be there for you, your family, and your loved one.
Our main goal of care coordination is to ensure that an individual’s care is consistent and comprehensive. Our coordinator will work with the family, their loved one, and their healthcare providers to develop a care plan that considers the individual’s needs and goals. Our coordinator will regularly review and update this care plan to ensure that the individual receives the most appropriate care
Our care coordinator can also help to reduce healthcare costs. Often, family members caring for a loved one can be overwhelmed and unable to find the time to keep up with the needs of their loved ones. We coordinate the various aspects of an individual’s care and ensure they are only receiving the care they need and not undergoing unnecessary or duplicative treatments.
In addition, when our care coordinator steps in, they can give all parties involved a sense of peace. Knowing that the weight is off of a loved one’s family allows the family to spend time with their loved one rather than jumping and dealing with all of the tasks that come with caring for a family member.
